Seamless Reimbursement: Dr. Febres provides comprehensive, automated superbills containing all necessary diagnostic and billing codes. Depending on your insurance plan, you may be reimbursed after your deductible is met.

Verifying your OON Benefits: Call the member services phone number on the back of your insurance card and ask the following:

1. Do I have out-of-network behavioral health benefits under my plan?

2. What is my out-of-network deductible for an individual and how much has been met so far this year?

3. What is the “allowed amount” or reimbursement percentage for:

   -CPT Code 90837 (individual therapy) OR

   -CPT Code 90847 (family or couples therapy), WITH

   -Modifier 95 (telehealth delivery)?

4. Do I need prior authorization to submit out-of-network superbills for reimbursement?

5. How do I submit superbills?

You are entitled to a Good Faith Estimate of the cost, number, and frequency of therapy sessions that can be reasonably expected for the anticipated services to address your mental health care needs.

The estimate is based on the information known to Dr. Febres when the estimate was completed. The estimate is subject to change depending on any changes over time to your treatment plan.
